FLIPA - Social Care Network Update

Join the conversation as Forward Leading IPA (FLIPA), the Social Care Network Lead Entity for the Finger Lakes region, provides an update on their progress in establishing the region's Social Care Network (SCN).

The creation of Social Care Networks (SCNs) is a key initiative under the NYHER Amendment, designed to enhance coordination of health-related social needs services across New York State. As part of this initiative, the State has designated nine regional SCN Lead Entities tasked with:

  • Building robust networks of Community-Based Organizations (CBOs) and other service providers.
  • Coordinating efforts with health care providers, including behavioral health and primary care.
  • Ensuring a seamless, consistent, and end-to-end process for Screening, Navigation, and service delivery within their regions.

Achieving these goals requires close collaboration within each Network, supported by shared data and technology provided by the State. SCN Lead Entities also work with a broader ecosystem of partners, including health insurance plans, local governments, and child and family support organizations.
