Position: CPT Success Coach
Reports to: Support Service Manager
Expected Pay range (based on experience): $55,000-$65,000/year
The Finger Lakes Performing Provider System (FLPPS) is a network of clinical and community-based providers working together across the 13-county Finger Lakes region to transform healthcare delivery, advance system transformation, close gaps in care, and support high-risk individuals and populations. FLPPS and its affiliated health home, the Greater Rochester Health Home Network (GRHHN) support providers in integrated healthcare delivery system optimization, care management, network and program management, population health and data analytics, and learning and development. Additionally, FLPPS is a designated New York State Workforce Investment Organization (WIO) working with training and educational institutions, health systems and other providers, and Social Care Networks to strengthen the healthcare workforce.
Position Summary
The CPT Success Coach plays a critical role in ensuring the success of participants enrolled in the Career Pathways Training (CPT) Program. This position provides holistic, individualized support to participants pursuing careers in health, behavioral health, or social care. The coach will guide participants through their educational programs, help them overcome barriers to success, and support their transition into employment with Medicaid-enrolled providers.
Key Accountabilities:
• Conduct regular check-ins with assigned participants to monitor academic progress, address challenges, and celebrate milestones.
• Develop individualized success plans that include academic, personal, and career goals.
• Connect participants with appropriate resources, including tutoring, mental health services, childcare, transportation, and financial aid.
• Collaborate with career counselors and employer partners to facilitate job placement aligned with participants’ training and service commitment.
• Support participants in preparing for interviews, resume development, and onboarding processes.
• Ensure participants understand and comply with the 3-year service commitment to Medicaid-enrolled providers.
• Maintain accurate and timely documentation in the WIO data/IT platform, including case notes, service plans, and employment verification.
• Track participant progress toward graduation and employment milestones.
• Assist with reporting requirements and contribute to program evaluation efforts.
• Work closely with educational institutions, workforce boards, and community-based organizations to coordinate services.
• Participate in team meetings, training sessions, and community outreach events.
• Serve as a liaison between participants and the CPT program team, ensuring alignment with program goals and policies
Required Qualifications:
• Bachelor’s degree in social work, counseling, education, public health, or a related field (Master’s preferred).
• Minimum of 2 years of experience in case management, academic advising, or workforce development.
• Demonstrated ability to work with diverse populations, including individuals from marginalized or underrepresented communities.
• Strong interpersonal, organizational, and problem-solving skills.
• Familiarity with Medicaid systems, workforce development programs, or health/social care sectors is a plus.
